Hotaru is a sweet child, but the power of Saturn is the power of death and rebirth, which makes her a little too powerful, which is scary. In addition to that, when Hotaru was in coma from an accident, her father made a pact with an evil force, which resulted in her being possessed. A possessed child is always scary.
The trope of a vampire child is not a new one. In 2001 Hellsing, Helena appears for a short period to be a creepy child. She is a powerful vampire, and she acts like an adult. She does not stay on the screen long enough before she is destroyed. She does not appear in Hellsing Ultimate because she is a part of the anime-exclusive plotline.
Another vampire child, Sunako is very different from Helena in that she does her best to act like a child. Perhaps, it is not all pretend, but she does seem to be the mastermind behind everything that happens in the village of Sotoba, and every vampire in the village is afraid of her. She uses childlike speech and mannerisms, and she does seem to be genuinely scared of dying, but she is the reason for a lot of killings, and she does look creepy, so she makes it on the list.
Creepy child is a common trope, especially in older anime, and while seeing one makes for a disturbing experience, sometimes we want anime to disturb us.
